| Description: | Introduction: Model evidence suggested that Hepatitis C Virus (HCV) treatment scale-up could reduce HCV prevalence and incidence among People who inject drugs (PWID). We aimed to generate empirical evidence on this HCV “Treatment as Prevention“ (TasP) hypothesis. Methods: Mixed methods study, including qualitative, statistical, mathematical and economic evaluation, of a natural experiment of HCV TasP in UK. Findings We demonstrated that rapid large-scale increase in HCV treatment in PWID could be successfully delivered in community settings, with Tayside exceeding local and WHO HCV care cascade targets for proportion of people with HCV diagnosed and treated. We enhanced HCV surveillance with Public Health Scotland to create new linked datasets: Substance Use and Health Intelligence Linked Dataset (SHIeLD) and developed novel methods - Multiple Parameter Estimation of Prevalence (MPEP) – using SHIeLD to estimate prevalence of people dependent on opioids used in our HCV models. We interviewed 40 service providers including peer workers, and 31 service users across the different community pathways in Tayside and co-produced a manual on how to scale-up treatment across the UK. We developed a statistical model for evaluating HCV TasP based on biobehavioural survey data of PWID in England and Scotland which generated trends in chronic HCV and the counterfactual (chronic HCV in absence of community scale-up of HCV treatment based on pre-intervention trends). We showed Scotland and England are likely to meet WHO elimination targets by 2030 and that community scale-up of HCV testing and treatment was highly cost-effective and might be costsaving. Qualitative interviews in 2023-24 with community teams in England found that normalising testing and treatment and peer support contributed to the success of the scale up. Implications Scaling-up HCV testing and treatment in the community and TasP is not only possible but highly effective – improving population health, reducing infections and saving NHS resources. |
