| Description: | Objectif: justifier scientifiquement l’opportunité d’étudier et d’analyser de manière critique l’expérience historique de l’organisation médicale militaire de l’armée américaine, pays leader de l’OTAN. Analyser les résultats de la modernisation radicale du système d’approvisionnement médical de l’armée américaine, menée par Jonathan Letterman au milieu des années1860. Souligner l’importance de ces innovations sur la base d’une étude approfondie et prouver leur pertinence dans le processus d’amélioration de l’organisation du soutien médical des forces armées ukrainiennes. Matériel et méthodes. La recherche a utilisé des documents provenant de publications en anglais consacrées à la formation et aux missions modernes de la médecine militaire américaine. Pour une analyse comparative de l’évolution historique de l’organisation médicale militaire des armées des continents américain et eurasien, les données des dernières publications du département de soutien médical des forces armées de l’Académie de médecine militaire ukrainienne sur le thème de la recherche et les publications du fonds de dépôt de la Bibliothèque nationale scientifique médicale d’Ukraine ont été utilisées. Méthodes appliquées d’analyse historique du système, de recherche d’informations et de bibliographie. Résultats. L'étude du développement du système de soutien médical de l'armée américaine a été présentée afin d'améliorer les activités des forces médicales des forces armées ukrainiennes. Les innovations de Jonathan Letterman dans la modernisation de la médecine militaire américaine durant la seconde moitié du XIXe siècle, qui font encore partie intégrante des opérations médicales militaires menées par l'armée américaine, sont mises en lumière et évaluées. Il est démontré que les principes fondamentaux de la médecine militaire, élaborés et introduits dans l'armée américaine dans les années 1860 par Letterman, étaient nettement en avance sur leur temps dans la compréhension des tâches prioritaires de la médecine militaire, conformément aux exigences des armées modernes. Une comparaison de l'efficacité de l'organisation médicale militaire des armées opérant sur les continents américain et eurasien durant cette période a été réalisée. Français Il a été établi que, contrairement à l'approbation législative et à l'introduction de changements novateurs dans l'organisation du soutien médical de l'armée américaine, le gouvernement autocratique de l'Empire russe a ignoré ou considérablement ralenti la modernisation de l'organisation médicale militaire de l'armée après la défaite de la guerre de Crimée (1853-1856), ce qui a eu des conséquences catastrophiques pendant la Première et la Seconde Guerre mondiale. L'importance du leadership dans la mise en œuvre de changements novateurs dans une composante aussi importante des forces armées que l'organisation médicale militaire est démontrée par l'exemple des activités de Jonathan Letterman. Conclusions : Le système de soutien médical des troupes sur le champ de bataille, développé et mis en œuvre par Jonathan Letterman dans la seconde moitié du XIXe siècle, reste aujourd'hui la base de la gestion des soins médicaux dans l'armée américaine en temps de guerre. La modernisation efficace de l'organisation médicale militaire de l'armée américaine pendant la guerre de Sécession (1861-1865) a eu un effet positif sur les capacités de la médecine militaire américaine au XXe et au début du XXIe siècle. Une compréhension critique de l’expérience historique de l’organisation du soutien médical dans l’armée américaine en tant que pays leader de l’OTAN est importante pour la croissance professionnelle des médecins militaires ukrainiens, ainsi que pour l’amélioration de l’organisation médicale militaire dans les forces armées ukrainiennes. Purpose: to scientifically substantiate the expediency of studying and critically analyzing the historical experience of the military medical organization of the US Army as a leading NATO country. To analyze the results of the radical modernization of the medical supply system of the American army carried out by Jonathan Letterman in the mid-1860s. Highlight the importance of these innovations on the basis of a comprehensive study and prove their relevance in the process of improving the organization of medical support of the Armed Forces of Ukraine. Materials and methods. The research used materials from English-language publications devoted to the formation and modern tasks of American military medicine. For a comparative analysis of the historical development of the military medical organization in the armies on the American and Eurasian continents, the data of the latest publications of the department of Medical Support of the Armed Forces of the Ukrainian Military Medical Academy on the subject of research and publications from the depository fund of the National Science Medical Library of Ukraine were used. Applied methods of system-historical analysis, information search, bibliographic. The results. Argued expediency of an study of the development of the medical support system of the US Army to improve the activities of the Medical Forces of the Armed Forces of Ukraine. Jonathan Letterman’s innovations in the modernization of American military medicine in the second half of the 19th century, which still remain an integral part of military medical operations implemented by the US Army, are highlighted and evaluated. It is proved, that the fundamental principles of army medicine, formed and introduced in the US army in the 1860s by Letterman, were significantly ahead of their time in understanding the priority tasks of military medicine in accordance with the requirements of modern armies. A comparison of the effectiveness of the military medical organization in the armies operating on the American and Eurasian continents during this period was made. It was established that, in contrast to the legislative approval and introduction of innovative changes in the organization of medical support of the US army, the autocratic government of the Russian Empire ignored or significantly slowed down the modernization of the military medical organization of the army after the defeat in the Crimean War (1853-1856), which led to catastrophic consequences during the First and Second World Wars. The importance of leadership in the implementation of innovative changes in such an important component of the armed forces as the military medical organization is proven on the example of Jonathan Letterman’s activities. Conclusions: The system of medical support of troops on the battlefield, developed and implemented by Jonathan Letterman in the second half of the 19th century, and today remains the basis of medical care management in the US Army in wartime conditions. The effective modernization of the military medical organization in the US army during the Civil War (1861‑1865) had a positive effect on the capabilities of American army medicine in the 20th and early 21st centuries. A critical understanding of the historical experience of the organization of medical support in the US Army as a leading country of the NATO is important for the professional growth of Ukrainian military doctors, as well as for the improvement of the military medical organization in the Armed Forces of Ukraine. Мета: науково обґрунтувати доцільність вивчення та критичного аналізу історичного досвіду військово-медичної організації армії США як провідної країни НАТО. Проаналізувати результати радикальної модернізації системи медичного постачання американської армії, проведеної Джонатаном Леттерманом у середині 1860-х років. Виділити значення цих нововведень на основі комплексного дослідження та довести їх актуальність у процесі вдосконалення організації медичного забезпечення Збройних Сил України. Матеріали та методи. У дослідженні використано матеріали англомовних видань, присвячених становленню та сучасним завданням американської військової медицини. Для порівняльного аналізу історичного розвитку військово-медичної організації в арміях на Американському та Євразійському континентах використано дані останніх публікацій кафедри медичного забезпечення Збройних Сил Української військово-медичної академії за темою дослідження та публікацій з депозитарного фонду Національної наукової медичної бібліотеки України. Застосовано методи системно-історичного аналізу, інформаційного пошуку, бібліографічного. Результати. Аргументовано доцільність дослідження розвитку системи медичного забезпечення армії США для покращення діяльності Медичних сил Збройних Сил України. Виділено та оцінено інновації Джонатана Леттермана в модернізації американської військової медицини у другій половині 19 століття, які досі залишаються невід'ємною частиною військово-медичних операцій, що здійснюються армією США. Доведено, що фундаментальні принципи армійської медицини, сформовані та впроваджені в армії США у 1860-х роках Леттерманом, значно випереджали свій час у розумінні пріоритетних завдань військової медицини відповідно до вимог сучасних армій. Здійснено порівняння ефективності військово-медичної організації в арміях, що діяли на Американському та Євразійському континентах у цей період. Встановлено, що, на відміну від законодавчого затвердження та впровадження інноваційних змін в організації медичного забезпечення армії США, автократичний уряд Російської імперії ігнорував або значно уповільнював модернізацію військово-медичної організації армії після поразки в Кримській війні (1853-1856), що призвело до катастрофічних наслідків під час Першої та Другої світових воєн. Важливість лідерства у впровадженні інноваційних змін у такому важливому компоненті збройних сил, як військово-медична організація, доведена на прикладі діяльності Джонатана Леттермана. Висновки: Система медичного забезпечення військ на полі бою, розроблена та впроваджена Джонатаном Леттерманом у другій половині 19 століття, і сьогодні залишається основою управління медичною допомогою в армії США в умовах воєнного часу. Ефективна модернізація військово-медичної організації в армії США під час Громадянської війни (1861‑1865) позитивно вплинула на можливості американської армійської медицини у 20-му та на початку 21-го століть. Критичне осмислення історичного досвіду організації медичного забезпечення в армії США як провідної країни НАТО є важливим для професійного зростання українських військових лікарів, а також для вдосконалення військово-медичної організації у Збройних Силах України. |
