The aim of this study was to develop a prosthetically driven classification for assessing the suitability of maxillary first premolars for immediate implant placement. A total of 150 CBCT datasets were evaluated using virtual implant planning based on the local osseous anatomy. A buccal bone thickness of <1 mm was found in 74 % of cases, while the distance to the maxillary sinus exceeded 3 mm in 79 %. Immediate implant placement was feasible primarily in the palatal root socket (56.6 %) or the interradicular septum (43.3 %); 24 % of sites were classified as unsuitable. The proposed classification may facilitate preoperative treatment planning. In cases with limited buccal bone thickness, grafting of the gap between the implant and the buccal bone wall is recommended. [ABSTRACT FROM AUTHOR] |

Ziel der im Beitrag beschriebenen Studie war die Entwicklung einer prothetisch orientierten Klassifikation zur Beurteilung der Eignung erster Oberkieferprämolaren für die Sofortimplantation. Anhand von 150 DVT-Datensätzen erfolgte eine virtuelle Implantatplanung unter Berücksichtigung der knöchernen Anatomie. Ein bukkaler Knochen <1 mm fand sich in 74 % der Fälle, ein Abstand von >3 mm zur Kieferhöhle in 79 %. Die Mehrzahl der Implantationen war im palatinalen Wurzelfach (56,6 %) oder im Septum interradiculare (43,3 %) möglich; 24 % der Fälle wurden als ungeeignet eingestuft. Die vorgestellte Klassifikation unterstützt die präoperative Planung. Bei geringer bukkaler Knochenstärke ist eine Augmentation des Spaltraums zu empfehlen. [ABSTRACT FROM AUTHOR] |
