Περίληψη: Therapeutic options for genetic conditions are rapidly expanding, including options for immediate postnatal therapy or in utero therapy. Many patient- and health care system-related factors influence access to and success of these therapies. At the most basic level, access to in utero therapies or immediate neonatal treatment is directly dependent upon access to prenatal counseling and diagnosis.1 As prenatal and postnatal diagnostic and therapeutic options continue to advance, the gaps in existing infrastructure for such specialized care delivery in the United States demand scrutiny. The purpose of this article is threefold. First, we will provide a general overview of the evolving landscape of prenatal diagnostic and therapeutic technologies. Second, we will review existing challenges to early, precise prenatal diagnosis and subsequent access to novel therapies and identify current ethical challenges that arise from introducing advancing technologies into the existing prenatal diagnostic health care structure. Finally, we will highlight opportunities to strengthen the existing US health care scaffolding to better incorporate novel therapeutics as they become more readily available.
