Περιγραφή: L’os es un òrgan complex, amb una estructura i composició determinades, anomenada “composta”, a més de presentar un alt metabolisme amb alt recanvi i remodelació de teixit. Aquest recanvi es troba alterat en algunes patologies com l’osteoporosi, fet que pot provocar fractures. Les fractures òssies poden ser degudes a altres causes com patologies relacionades amb el dèficit de vitamina D i calci, amb la nutrició, hormonals, metabòliques o autoimmunes, incloent l’aparició de tumors i accidents. Aquesta capacitat de degradar i generar nou os, permet la regeneració de fractures menors a 2.5cm, mentre que fractures majors no permeten la regeneració del teixit per les cèl·lules i molècules del propi os, per això, en aquests casos es necessari incorporar material extern per a regenerar. Inicialment s’utilitzaven implants d’os provinents del mateix pacient o d’altre pacient o espècie, però el seu us causava inflamació, poca osteointegració i rebuig. Per a posar fi a aquests desavantatges, es van començar a fabricar implants artificials. L’enginyeria de teixits es centra en la fabricació d’implants per a diferents teixits combinant bastides, factors i cèl·lules. Els implants artificials d’os han de ser biocompatibles, biodegradables, bioactius, interaccionar amb el propi os afavorint la formació de teixit nou, amb una porositat adequada i propietats mecàniques similars a les de l’os. A més, han de mimetitzar l’estructura de l’os i la seva composició, que consta d’una part inorgànica de cristalls d’hidroxiapatita (HA) i una part orgànica formada per fibril·les de col·lagen tipus I. Tots aquests requisits es poden aconseguir amb la fabricació de bastides amb biomaterials al laboratori. Es poden fabricar amb morfologia de fibres, impresos 3D, microesferes, cilindres i films. Els biomaterials més adequats per a mimetitzar els cristalls d’hidroxiapatita son els ceràmics, per la seva naturalesa bioactiva, capaç d’induir la formació d’HA i per la seva rigidesa. El col·lagen tipus I, tot i en poques quantitats, té un cost elevat, per això s’utilitza un derivat del col·lagen provinent de la seva hidròlisi, com es la gelatina, que manté les seqüències RGD, favorables per a l’adhesió cel·lular. La combinació d’aquests dos tipus de biomaterials amb diferents morfologies poden ser prometedors per a fabricar implants sintètics per a la regeneració de l’os. Com s’ha explicat anteriorment, el teixit hostatger, presenta diferents molècules i factors que ajuden a les cèl·lules a regenerar l’os. Per tant, la incorporació de factors en les bastides pot ser una bona estratègia per a millorar la regeneració de l’os.
Description (Translated): Bone is a complex organ with a defined composite structure and composition as well as a high metabolism with bone turnover and remodeling. This bone turnover is altered in some pathologies as osteoporosis, which can lead to fractures. Fractures can be caused by pathologies related to vitamin D and calcium deficiency, nutritional, hormonal, metabolic or autoimmune diseases, tumors or accidents. This capacity to degrade and generate new tissue allows the regeneration of certain damages of a size lower than 2.5cm. Bigger damages cannot be regenerated by the host cells and molecules, so external material is needed to be incorporated to the body. Initially, bone grafts were natural tissue from the same patient or a donor from the same specie or another compatible specie, but there was inflammation, poor osseointegration and rejection. To overcome all these drawbacks, synthetic grafts started to be fabricated for its implantation. Tissue engineering focuses on the fabrication of grafts for different tissues by combining scaffolds, factors and cells. Synthetic bone grafts should be biocompatible, biodegradable, bioactive, osteoconductive and osteoinductive, with proper porosity and mechanical properties. In addition, they should mimic bone structure and composition of an inorganic phase of hydroxyapatite (HA) crystals and an organic phase of collagen type I fibrils. All these requirements can be fulfilled by the fabrication of scaffolds in the laboratory with biomaterials. They can be fabricated in the morphology of fibers, 3D printed scaffolds, microspheres or cylinders and films. The most adequate biomaterials to mimic the hydroxyapatite crystals are ceramics for their bioactive nature capable of inducing HA formation and rigidity. Collagen type I has a high cost for low quantity, so a collagen derivative from its hydrolysis is used, i.e. gelatin, that maintains the RGD sequences favorable for cell adhesion. The combination of these two types of biomaterials with different morphologies could be promising to fabricate synthetic grafts for bone regeneration. As explained, host tissue presents different molecules or factors that help cells in bone regeneration, so the incorporation of factors in the scaffolds could be a good strategy to enhance bone regeneration.
