Description (Translated): The use of high-throughput screening platforms have revealed the existence of several transcriptomic subtypes of glioblastoma (GB) that define patients’ subgroups with different survival and response to therapy. One of the most referenced published models is that of Verhaak et al, which divides GB into the proneural (PRN), neural (NEU), classic (CLS), and mesenchymal (MES) subtypes. In addition to different gene expression pattern, these subtypes show different gene mutations and copy number variations, among others. The technologies used in these studies are very complex and expensive and, therefore, are not practical tools for routine GB classification. The objective of the present work was to reproduce this transcriptomic classification in a simpler and more economic manner by using techniques that are widely available in pathology departments. In a series of 64 GB cases we assessed immunohistochemical (IHC) expression of 11 markers (IDH1-R132H, p53, PTEN, CD44, Olig2, p16, D2-40, PDGFRA, vimentin-VIM, glial fibrillary acidic protein-GFAP, and synaptophysin-SYN), as well as EGFR and PDGFRA amplification by FISH and EGFRvIII occurrence by RT-PCR. With the data obtained we have tested two different classification approaches. On the one hand, we have applied a consensus clustering methodology that has allowed us to identify three GB subgroups (A, B, and C) that mimic PRN, CLS, and MES Verhaak subtypes, respectively. On the other hand, we have developed a "decision tree" classification algorithm based on the sequential analysis of IDH mutations, EGFRvIII occurrence, PDGFRA amplification, p53 and p16 IHC expression, EGFR amplification, and D2-40 and CD44 IHC expression. The “decision tree” approach allowed us to distinguish three GB subgroups (GB-P, GB-C, and GB-M) which also mimic the PRN, CLS, and MES Verhaak subtypes, respectively. The GB-P, GB-C, and GB-M subgroups show statistically significant differences in overall survival. Specifically, patients with GB subgroup GB-C (which mimics the CLS Verhaak subtype) have longer survival than patients in the other subgroups. In addition, belonging to the GB-C subgroup is a good prognostic factor, independently of the type of surgery, age, and Karnofsky index. There is a statistically significant association between the GB subgroups generated by our two models (the consensus clustering model and the “decision tree” type model). In contrast to the consensus clustering model technical difficulties, the "decision tree" classification algorithm would be a sufficiently precise and easily implemented tool for routine GB classification. Finally, in our hands, synaptophysin immunoreactivity is not sufficiently reliable to be considered as a marker of the NEU subtype of GB, a subtype whose existence we doubt.
