Description: Introduction. Macrolides are essential chemotherapeutic agents with bacteriostatic and bactericidal effects in the treatment of chronic inflammatory conditions. This article explores their mechanisms of action and therapeutic potential in diseases such as non-cystic fibrosis bronchiectasis, asthma, and diffuse panbronchiolitis. Materials and Methods. To evaluate the role of macrolides in the treatment of chronic inflammatory diseases, a systematic literature review was conducted using databases such as PubMed Central, Cochrane Library, Medline, and Google Scholar. The studies included were selected based on relevance and quality, with a particular focus on the mechanisms of action described in the context of the studied conditions. Results. The analysis of the articles demonstrated that macrolides exert various effects on the immune system and the inflammatory process. These include, but are not limited to, modulation of the immune response, inhibition of pro-inflammatory cytokine production, and reduction of pathogen activity through both direct and indirect protective mechanisms. Conclusions. Macrolides prove to be valuable tools in the management of chronic inflammatory diseases, offering not only an antimicrobial effect but also anti-inflammatory propertie s that can improve patientsrsquo; quality of life. This synthesis highlights the need for further research to optimize the use of macrolides in clinical practice.
Introducere. Macrolidele sunt agenți chimioterapici importanți, cu efecte bacteriostatice și bactericide, utilizate în tratamentul unor afecțiuni inflamatorii cronice. Acest articol explorează mecanismele lor de acțiune și potențialul terapeutic în boli precum bronșiectazia non-fibrochistică, astmul și parabronhiolita difuză. Materiale și metode. Pentru a evalua rolul macrolidelor în tratarea bolilor inflamatorii cronice, a fost realizată o revizuire sistematică a literaturii, utilizând baze de date precum PubMed Central, Cochrane Library, Medline și Google Scholar. Studiile incluse au fost selectate pe baza relevanței și calității, cu un accent special pe mecanismele de acțiune descrise în contextul afecțiunilor studiate. Rezultate. Analiza articolelor a demonstrat că macrolidele exercită efecte variate asupra sistemului imun și procesului inflamator. Acestea includ, dar nu se limitează la, modularea răspunsului imun, inhibarea producției de citokine proinflamatorii și reducerea activității agenților patogeni prin mecanisme de protecție directe și indirecte. Concluzii. Macrolidele se dovedesc a fi instrumente valoroase în gestionarea bolilor inflamatorii cronice, oferind nu doar un efect antimicrobian, ci și proprietăți antiinflamatorii care pot îmbunătăți calitatea vieții pacienților. Această sinteză subliniază necesitatea unor cercetări suplimentare pentru a optimiza utilizarea macrolidelor în practica clinică.
Введение. Макролиды являются важными химиотерапевтическими агентами с бактериостатическими и бактерицидными эффектами, применяемыми в лечении хронических воспалительных заболеваний. В данной статье рассматриваются механизмы их действия и терапевтический потенциал при таких заболеваниях, как некистозная бронхоэктазия, астма и диффузный парабронхиолит. Материалы и методы. Для оценки роли макролидов в лечении хронических воспалительных заболеваний был проведен систематический обзор литературы с использованием таких баз данных, как PubMed Central, Cochrane Library, Medline и Google Scholar. Включенные исследования были отобраны на основе релевантности и качества, с особым акцентом на механизмы действия, описанные в контексте изученных состояний. Результаты. Анализ статей показал, что макролиды оказывают различное воздействие на иммунную систему и воспалительный процесс. Они включают: модулирование иммунного ответа, ингибирование продукции провоспалительных цитокинов и снижение активности патогенов посредством прямых и косвенных защитных механизмов. Выводы. Макролиды являются ценным инструментом в лечении хронических воспалительных заболеваний, обеспечивая не только противомикробный эффект, но и противовоспалительные свойства, которые могут улучшить качество жизни пациентов. Это резюме подчеркивает необходимость дальнейших исследований для оптимизации использования макролидов в клинической практике.
