Περιγραφή: Closed abdominal trauma is one of the main reasons for urgent hospitalization in a surgical hospital and subsequent surgical treatment. The most severe condition is damage to the pancreas due to the development of complications. Objective — to present a clinical case of a patient (age of 19) with closed abdominal injury and pancreatic injury. Materials and methods. The patient was crushed by a heavy load at work. The diagnosis was made using radiological examination methods. Laparotomy was performed. The postoperative period was complicated by the course of posttraumatic pancreatitis, which required programmed relaparotomies and the formation of an external pancreatic fistula. Results. After 2 months of admission, taking into account the positive dynamics in treatment, the patient was discharged under the supervision of a surgeon and traumatologist. The article also discusses the treatment of this category of victims. Conclusion. The difficulties of diagnosing and treating pancreatic injuries in blunt abdominal trauma remain one of the most challenging issues in emergency surgery. The most common complication of pancreatic trauma is posttraumatic pancreatitis with the formation of pancreatic fistulas, which complicates the postoperative period and worsens treatment results. Management in in conditions of the multi-profile hospital with the use of the whole complex of medical and diagnostic procedures is the main factor for decrease in mortality and incidence of complications after pancreatic injury.
Закрытая травма живота — одна из основных причин ургентной госпитализации в хирургический стационар и последующего оперативного лечения. Наиболее тяжело протекает повреждение поджелудочной железы в связи с развитием осложнений. Цель — представить клиническое наблюдение пострадавшего 19 лет с закрытой травмой живота с повреждением поджелудочной железы. Материалы и методы. Пациента придавило тяжелым грузом на рабочем месте. Диагноз поставлен благодаря использованию лучевых методов исследования. Выполнена лапаротомия. Послеоперационный период осложнился течением посттравматического панкреатита, что потребовало проведения программированных релапаротомий, формирования наружного панкреатического свища. Результаты. Через 2 месяца после поступления с учетом положительной динамики в лечении пациент выписан под наблюдение хирурга и травматолога. В статье также обсуждается лечение данной категории пострадавших. Заключение. Трудности диагностики и лечения повреждений поджелудочной железы при закрытой травме живота остаются одним из самых сложных вопросов ургентной хирургии. Наиболее частым осложнением травмы поджелудочной железы является посттравматический панкреатит с формированием панкреатических свищей, что осложняет послеоперационный период и ухудшает результаты лечения. Лечение в условиях многопрофильного стационара с использованием всего комплекса лечебно-диагностических процедур — основной фактор снижения летальности и частоты осложнений при травме поджелудочной железы.
