| Description: | Представлены результаты внедрения новых видов эндохирургических операций на предстательной железе при ее доброкачественной гиперплазии (ДГП). Сравнение проведено между двумя группами пациентов, оперированных в отделении урологии Дорожной клинической больницы г. Иркутск методами биполярной трансуретральной резекции и энуклеации простаты (44 пациента) и открытой аденомэктомии (47 пациентов), применяемой для хирургического лечения ДГП до внедрения новых методов операций. Сравниваемые группы пациентов были полностью сопоставимы по основным исходным параметрам (объем простаты, урологические показатели, осложнения основного заболевания и сопутствующая патология). Установлено, что при статистически равном объеме удаленной ткани, объем кровопотери во время операции, сроки послеоперационной катетеризации мочевого пузыря и длительность пребывания в стационаре при использовании новых методов операции было более чем в 2 раза меньше. Количество осложнений интраоперационного и раннего послеоперационного периодов после трансуретральных вмешательств было меньшим, соответственно в 2,8-3,4 раза. По отсроченным осложнениям и показателям эффективности оперативного вмешательства на сроке контрольного наблюдения через 6 месяцев после операции статистически значимой разности между сравниваемыми группами пациентов не выявлено. Установленные преимущества свидетельствуют о важности проведенного внедрения как с точки зрения улучшения клинических результатов и более быстрого восстановления функционального состояния и качества жизни пациентов, так и с позиции экономии средств стационара за счет сокращения сроков госпитализации. The results of the introduction of new types of endosurgical interventions on the prostate gland in its benign hyperplasia (BPH) are presented. A comparison was made between two groups of patients operated in the urology Department of the Irkutsk Road clinical hospital using bipolar transurethral resection and enucleation of the prostate (44 patients) and open adenomectomy (47 patients) used for surgical treatment of BPH before the introduction of new methods of interventions. The compared groups of patients were completely comparable in terms of the main initial parameters (prostate volume, urological indicators, complications of the main disease and concomitant pathology). It was found that with a statistically equal volume of removed tissue, the volume of blood loss during the intervention, the timing of postoperative catheterization of the bladder and the length of hospital stay when using new methods of operation was more than 2 times less. The number of complications in the intraoperative and early postoperative periods after transurethral interventions was 2.8-3.4 times lower, respectively. For delayed complications and indicators of the effectiveness of surgical intervention at the time of control observation after 6 months after the operation, no statistically significant difference was found between the compared groups of patients. The established advantages indicate the importance of implementation both in terms of improving clinical results and faster recovery of the functional state and quality of life of patients, and from the point of view of saving hospital funds by reducing the time of hospitalization. |
