| Abstract: | Background: In the assessment of personality disorders, the growing popularity of the dimensional approach (DSM-5 AMPD, ICD-11) has led to an increased emphasis on the role of psychodynamically based structural diagnosis. The key question is how the level of deeper personality organization (STIPO-R) relates to surface complaints (MMPI-2) and whether it can predict therapeutic outcome independently of symptom severity. Objective: The aim of our research is to explore the relationship between the level of personality organization and psychopathological symptoms, and to examine whether the level of personality organization predicts symptomatic change during psychotherapeutic treatment. Method: We conducted STIPO-R interviews with adult patients (N=157) treated at the Tündérhegy Psychotherapy Department upon admission, as well as MMPI-2 questionnaires at the beginning and end of treatment. We analyzed the relationship between the variables using correlation tests and applied hierarchical linear regression analysis to control for the statistical effect of "regression to the mean." Results: The STIPO-R dimensions showed moderate convergent validity with the MMPI-2 psychopathology scales. When analyzing therapeutic change, controlling for initial symptom severity, higher STIPO-R scores (lower organizational level) predicted significantly smaller improvements in Negative Emotionality (NEGE) and Psychoticism (PSYC). Conclusion: The STIPO-R goes beyond symptom description: it can predict therapeutic resistance, which may be a consequence of structural deficits (e.g., difficulties with affect regulation and reality control). For patients with lower levels of personality organization, structural diagnosis can help set realistic therapeutic goals and emphasize personality disorder-specific interventions (e.g., transference focus).
