Abstract: A number of key misidentification risk points occur during an in vitro fertilization cycle in the laboratory that require robust witnessing. Witnessing protocols, both manual and electronic, are described. In the event of a protocol deviation, guidance for grading, reporting, and risk mitigation is provided.
