This article addresses methodological considerations in the analysis of biological variation for personalized reference intervals (prRIs) in laboratory medicine. It clarifies that prRIs are derived from a single individual's longitudinal data, where both within-person biological variation and analytical variation are inherently combined, making the use of standard deviation (SD) or coefficient of variation (CV) mathematically equivalent for estimating prRIs. The authors argue that CV is preferred in broader laboratory medicine contexts due to its relative stability across concentration ranges, unlike SD which is concentration-dependent. They also emphasize that biological variation components—analytical variation (CVA), within-subject variation (CVI), and between-subject variation (CVG)—are hierarchically structured and derived from the same dataset, thus total variation should be normalized by the mean (m), not by three times the mean (3m). Finally, the article contends that prRIs better reflect individual metabolic dynamics than population-based reference intervals (popRIs), which may not be appropriate for individual clinical decision-making.
